Insurers organize temporary cover into rate tiers. Names shift by vendor, yet the rank holds: Preferred Plus (Super Preferred, Elite), Preferred, Standard Plus, Standard, then premium hikes for elevated risk. Smokers occupy a separate, pricier rung.
What underwriters look at
Frame (height and weight), BP and lipids, relatives' cardiac or cancer episodes before 60, driving past, meds taken, work and pursuits, and nicotine status. Top tiers want near-perfect marks on all; Preferred forgives some slack; lower tiers OK managed illnesses like regulated high BP.
